A few reminders:
Mistreatment by a patient is generally handled through the workplace safety procedures designed to protect health care professionals (available from your Clerkship/Selective/Adv Clerkship Director and/or on the Clerkship Resources site or via the NYU Langone Health clearinghouse for policies/procedures: http://nyumc.ellucid.com ). However, being mistreated by patients can be distressing especially without acknowledgment/support and so we encourage you to discuss with your clinical supervisors as they are committed to supporting learners when these incidents happen. Should you wish to report mistreatment by a patient because it created a negative learning environment, was distressing, and/or was not addressed adequately in terms of support of students, we have included the option for doing so in this form -- please select the "Mistreatment by a Patient" category.

However, you are always encouraged to contact your Module or Clerkship Director, education leadership (e.g., the Associate Dean for Student Affairs, the Associate Dean of Curriculum, the Assistant Dean Director for Education in the Basic Sciences and the Assistant Dean for Education in the Clinical Sciences), Pre-Clinical/Clinical Student Liaisons, Faculty Members/Advisors, and Supervising Residents to discuss mistreatment and receive guidance in reporting mistreatment.
This form itself is ANONYMOUS -- the link you clicked to get here cannot and does not store any identifying information. However, the form does ask you if you would like to provide your name and contact information in order to further resolve the incident -- you can choose to do so or not. If you choose to provide your name, your information is kept strictly confidential (within limits determined by safety concerns). If you choose NOT to provide your name, your report is anonymous (no identity data are collected and there is no way within the program to link a specific record to an individual). Please note that anonymous reporting limits our ability to respond and resolve the specific incident you are reporting -- however, we will track and monitor such incidents to prevent further problems. If you report an incident confidentially, access to your identity will be limited to evaluation personnel (myself - the Director of Evaluation and designee) and to education leadership (the Associate Dean of Student Affairs and designee and the Assistant Dean for Education in the Clinical Sciences or the Director of Education in the Basic Sciences, depending on curricular stage).
If you provided your name, unless safety concerns require it, your identity will not be shared with anyone else without your permission. Specific details of an incident are not shared with individual Clerkship or Module Directors until after grades/evaluations are completed (and even then only at the level of detail with which you are comfortable, unless safety concerns over-ride). Additionally, you may request that response to the incident be delayed until a time when you are comfortable with the initiation of the response (e.g., after your clerkship year, at graduation, etc).
If you wish to be contacted about the incident, your name will be forwarded to the Associate Dean for Student Affairs who will then reach out to you. The Associate Dean for Student Affairs then works with Assistant Dean for Medical Education in the Basic or Clinical Sciences (as appropriate depending on the curricular stage) to review the report and determine the most appropriate response.
PLEASE NOTE: Responses to reported incidents are handled with care and titrated to the seriousness of the incident. Feedback is given, when appropriate and at the level of confidentiality agreed upon, in a growth-oriented, positive frame with a focus on providing support and resources, and not on punishment. If the incident is determined to be sufficiently serious, the individual may be removed from their education role or be subject to other employment-related consequences. As the Director of the Division of Education Quality, my role is to provide students with a secure method for reporting concerns about the learning environment and then tracking the actions taken in response to those concerns. The Associate Dean for Student Affairs will review this report and work with the the relevant individuals (e.g., Director of Education in the Basic Sciences, Assistant Dean for Education in the Clinical Sciences, Curricular Directors, Faculty, Residency Program Directors, Department Chairs etc) to determine the appropriate response.
Aggregate, non-identified data on mistreatment reports and the response to those reports is shared at least annually with the Curriculum Committee to monitor trends and ensure that action is being taking to prevent and minimize mistreatment. Every six months, fully de-identified, general descriptions of reported incidents and responses are summarized in a report and made available to students (via the Learning Environment and Mistreatment reporting page in Brightspace, "You Said, We Heard, We Did", and through Town Halls, Student Council Meetings, Pre-Clerkship and Clerkship Student Liaisons).
